PLEASE NOTE

You may be required to answer one or more of these questions again at the time of your donation. In some cases, you may be required to answer one or more questions again with your Donor Technician at the time of your donation.

You may also choose to review a question with your Donor Technician for clarification.

REMINDERS

Please have your ID ready! As a friendly reminder from The Community Blood Center, we will ask you to provide a positive form of ID during the donation process.
Attention 16-year-old donors: please have your signed parental/guardian consent ready! We will ask you to provide a signed parental/guardian consent form prior to donation.
Bring your donor receipt. Upon completion of READYDONOR, you can choose to print, save, or email your receipt that contains your QR code. Without a receipt, you will be required to answer the questions again at your donation.
